Clinic referrals

A referral from GP or specialist is required.

Outreach referrals

We accept referrals from people and their families, GPs and other medical specialists, health professionals and community service providers.

About our service

We see adults with chronic and complex lung conditions who:

• also have other medical conditions and would benefit from an appointment with a respiratory specialist and can be followed up at home by a respiratory outreach nurse.
• are at risk of hospital presentation and admission or have had frequent hospital admissions.
• have a carer who needs support.

We provide:

• longer respiratory specialist appointments, allowing time to discuss complex issues.
• a multidisciplinary team in clinic.
• access to other HIP services.
• an outreach service to reinforce key messages regarding the self-management of chronic disease.

Current wait times

The waitlist for a clinic appointment is approximately 1 to 3 months however all referrals are triaged, and we will aim to see more urgent referrals in 4 to 6 weeks.
