About your Health Questionnaire
When you are added to the waiting list for a planned procedure at Austin Health, we will ask you to complete your Health Questionnaire.
Your Health Questionnaire is a detailed form that collects information about your medical history. It covers:
- past procedures
- medications
- allergies
- and other relevant health details.
This makes sure your care team has all the information they need to safely plan your procedure and provide you with the best care.
We'll also ask you to update your form from time to time to help us detect any changes early and ensure they don't impact your care.
Why it is important
Completing your Health Questionnaire helps us to:
- reduce risk: by knowing your medical history and any potential concerns, we can better prevent complications during your stay.
- provide personalised care: the form offers insights that allow your care team to tailor your care plan to your individual needs.
- improve efficiency: helps our staff prepare for your arrival, ensuring all necessary resources are available and reducing the chance of cancellations.
How to complete your form online
You can complete your Health Questionnaire online using our Patient Portal.
Austin Health is using VicKey (formerly MyHospital) to deliver the Patient Portal service.
Set up your account
You will need a VicKey account to access your Health Questionnaire through our Patient Portal.
To set up your account you will need to have your UR number (patient identification number) and mobile phone handy.

Find the form in your inbox
You will find your Health Questionnaire in your Patient Portal inbox.
Before you start
- Allow at least 60 minutes to complete all sections of the form.
- Have your medical information ready, such as Medicare details, current medications, and past medical conditions.
- You can save your progress and return later if needed, or complete it with someone’s help.
- To avoid delays in your care journey, please submit the completed form as soon as possible.

Completing your Health Questionnaire as a paper form
The Patient Portal is an optional service. If you wish to use a paper form to complete your Health Questionnaire, you can let us know by contacting the Patient Portal Support Office.
