About us

We aim to help you manage your medications independently. We work closely with your GP and community pharmacist.

We have a team of pharmacists who can arrange a single home visit to help people recently discharged from hospital to manage their medications. 

Our services

We provide the following services:

• Manage complex medication regimes
• Provide further education about medications
• Manage known or suspected medication issues
• Liaise with GPs and community pharmacists

Referrals

You can access this service when you are staying in hospital. Ask a member of your care team to contact us.

If a patient is well known to another health service, the Medication Management team may refer the patient onto a similar service at another hospital.
