About us
The Transition Care Program is designed for older people who may benefit from further therapy or supports after a hospital admission, to help them to continue to live independently.
The program also supports people who are considering longer-term care arrangements, including residential care.
We provide care and support for:
- People who are 65 years and over
- Aboriginal and/or Torres Strait Islander peoples who are 50 years and over
The Transition Care Program can be delivered in your home, or in a specific residential care facility.
Austin Health partners with Twin Parks Aged Care Centre to deliver bed-based transition care.
If your needs change, you can move between settings. For example, you might begin transition care in a residential care facility, and then go home to continue your recovery, with supports in place in your own community.
Our services
We provide the following services, depending on your needs:
- Case management
- Medical support
- Allied health (for example, physiotherapy, occupational therapy)
- Personal care assistance
- Nursing services
- Continence aids
- Equipment hire
- Domestic home care
Referrals
Access to the Transition Care Program is usually arranged by your treating team while you are in hospital, after discussing this with you and your family or carers.
